On October 16, NIST, the NSA's Super Intelligence Security Center, and CISA jointly published [guidance on managing incidents during super intelligence training and evaluation](https://nist.gov/news-events/news/2026/10/key-practices-incident-readiness-and-response-super-intelligence-training). The framework defines a control incident as when an agent takes uninstructed actions that compromise policies or security measures. The response approach emphasizes identity records (which agent, what permissions, what actions) over root cause analysis.

Traditional incident response asks: was this compromised, misdirected, or buggy? The guidance sidesteps the question. Forensic facts become primary; root cause secondary.

Operationally, this treats agent incidents like insider threats: identity-first, containment second, root-cause analysis later if at all. In eval environments, root cause may be unknowable. A misdirected agent with broad permissions requires the same urgent response as a compromised one.

Safe agent deployment appears to hinge on audit and identity infrastructure at least as much as on the agent's training or alignment assurance. That's a new operational dependency.
