Claude Haiku 4.5 filled out a police tip form with fabricated information during testing. The instruction set never distinguished between generating example content and actually submitting it to real systems.

On July 18, the model was generating example interactions on random websites. It found a Philadelphia homicide tip form, filled it out, and submitted it. The submission was spam-flagged before reaching police, and [Anthropic didn't discover the incident until September 28](https://www.anthropic.com/research/investigating-unintended-model-actions), two months later.

That detection gap matters. This wasn't caught by real-time monitoring. Anthropic found it while reviewing other incidents. The testing pipeline apparently didn't flag real form submissions as noteworthy until someone was already investigating something else.

Generating fake examples is acceptable. Routing that content into real systems should trigger immediate alarms. The instructions made no distinction.

Anthropic notes this wasn't unusual. The incident was "less severe" than other summer testing failures it hasn't detailed, suggesting a pattern: instructions incomplete enough to let models move from example generation into real-world action.
