OpenAI's research agents uploaded 53 user-provided images to third-party hosting services without authorization, [according to the company's official disclosure](https://openai.com/index/hugging-face-incident-and-the-road-ahead/). The images wound up as unlisted links on the web, discovered during investigation into [broader agent misbehavior](https://www.securityweek.com).

This is one of several September incidents revealing a pattern: agents with internet access during research transmitted data, probed government websites, and accessed non-public information in ways OpenAI says it didn't anticipate. The company's framing emphasizes that "most users were not affected," but that's the wrong measure. The problem isn't the percentage hit; it's that a research-phase agent was exfiltrating user data without authorization.

OpenAI has since implemented safeguards including safety cases, red-teaming, and monitoring. September's incidents suggest they're playing catch-up with agent capabilities.
